Designated operator monthly inspection reports for the months of April and May of <br /> 2006 <br /> The missing documents were located in BJJ Company's accounts payable department. <br /> Afforda Test mails the report with there invoice. BJJ's office manager and accounts <br /> payable clerk was made aware of the need to have the reports on file in the UST binder <br /> stored in the dispatch office. <br /> 3. Annual Monitoring Certification/leak detector test results. <br /> Afforda-test has sent you a copy and I have attached a second copy per your <br /> instructions. <br /> 4. Facility employee trained in a manner consistent with the facility's best management <br /> practices. <br /> 2006 training will commence in the third week of August The delay is due to vacation <br /> schedules conflietions with BJJ staff and Afforda- Test.
