The EHD will generate and provide a complete report to the facility operator. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, within 30 days of receiving the complete inspection report. Note: Communication in under dispenser containment 5-6 was verified by a port located near the brine reservoir. Communication needs to be tested away from the brine reservoir. Immediately contact a properly licensed, trained, and certified contractor to install the necessary component at the far end of the under dispenser containment 5-6, away from the reservoir. Note: A boot in the 87-product submersible turbine pump sump and a boot in the 91-product fill sump were broken. Immediately contact a properly licensed, trained, and certified contractor to repair/replace the broken boots under permit and inspection by the EHD.
