The plans list both a TLS 350 and a TLS 350R <br /> for the monitoring panel on site. The panel observed is a TLS 350R. The monitoring plans must be uploaded to the <br /> California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and <br /> submit for review by the EHD. <br /> This is a repeat violation, Class II. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> The 87 STP sump chase pipe boot and the 91 fill sump chase pipe boot were replaced on 8/14/17 and a permit was <br /> not obtained from the EHD. Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained <br /> within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD <br /> prior to beginning work. Immediately obtain a permit from the EHD for this work. A"Failure to Obtain a Permit" <br /> penalty applies. *A permit application was submitted on June 4, 2018, but has not yet been completed. <br /> This is a repeat violation, Class II. <br /> 301 HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. The <br /> underground storage tank system shall be operated to prevent unauthorized releases. Immediately provide approved <br /> secondary containment for the filters under permit and inspection by the EHD. <br /> This is a Class II violation. <br /> 305 HSC 25290.1(e) VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> Boots on the chase piping in the diesel STP sump, diesel fill sump, 91 STP sump, and 87 fill sump were observed <br /> broken, exposing the ends of the open piping. No vacuum sensors were monitoring the secondary chase piping. The <br /> interstitial space of the UST shall be maintained, as designed, under constant vacuum such that a breach in the <br /> primary or secondary containment is detected before the hazardous substance is released into the environment. <br /> Correct immediately by having a properly licensed, trained, and certified contractor replace the failed component with a <br /> functional component(LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the <br /> failed component can't be replaced immediately, there is a possibility that the 87, 91, and diesel UST systems may <br /> be red tagged to prevent fuel inputs. <br /> This is a Class II violation. <br /> 314 HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br /> The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. Secondary <br /> containment shall be provided for all single-walled UST components.
