Each of the issues have been addressed as noted <br /> below: <br /> Issue#1: <br /> • Secondary containment testing was done in 3/29/04. This office does not have a copy of this test <br /> result. Submit a copy to this office immediately. <br /> Response#1: <br /> • The Secondary Containment testing was conducted by Wayne Perry, Inc. (see attached) <br /> Issue#2 <br /> • This office does not have a copy of the Certification of Financial Responsibility. Provide a copy to <br /> this office. <br /> Response#2 <br /> • A copy of the Certificate of Financial Responsibility was submitted to your office by US mail on <br /> 3/3/06. (copy attached) <br /> Issue#3 <br /> • Rudy Tovar, according to the facility's monthly inspection reports. This office does not have the <br /> owner's statement form naming him as the designated operator. Fill out form and submit a copy to <br /> this office immediately. <br /> Response#3 <br /> • The Designator Operator for this facility is Leslie Palmer. (see change of D.O. form) <br /> Issue#4 <br /> • The D.O. must train all facility employees. <br /> Response#4 <br /> • The facility employee training was conducted by Laurie Jones. (see attached) <br /> Issue#5 <br /> • Monitoring certification was performed on 5/5/06. Submit results to this office immediately. <br /> Page 1 of 2 <br />
