This paperwork is being submitted to update either the <br /> certification numbers of the DO (previously we only had the paperwork from the testing facility <br /> showing personnel passed the test, which did not include a certification number), to add DO <br /> alternates, and/or to change the primary DO for the facility. <br /> It is my understanding based on guidance from State Water Resources Control Board (SWRCB) <br /> at the CUPA conference that the certification of compliance is not required for updates to DO <br /> information; therefore, this statement has been removed from the form.
