Each issue has been addressed as noted below: <br /> Issue#1: <br /> • Please submit 2004 Financial Responsibility no later than 4/8/04. <br /> Response#1: <br /> • A copy of the Certificate of Insurance, which is valid December 17, 2003 through December 17, 2004, <br /> was faxed to you on March 9, 2004. <br /> Issue#2: <br /> • The technician cancelled the monitor certification due to illness. Reschedule and conduct the monitor <br /> certification by 6/5/04. <br /> Response#2: <br /> • Our testing contractor, Triangle Environmental, has rescheduled the monitor certification for 3/31/04 at <br /> 10:30 a.m. <br /> Issue#3: <br /> • Annual spill bucket testing must be conducted and results submitted by 12/31/04. <br /> Response#3: <br /> • This testing will be conducted as required. <br /> Issue#4: <br /> • Monitoring records not being kept. <br /> Response#4: <br /> • The store manager maintains an alarm log for the monitoring system. I spoke to the manager, David <br /> Hall, and he indicated that he was previously only recording the alarms for the monitoring system. He <br /> has immediately begun documenting the operation of the monitoring system each day by recording <br /> .no alarms"on days when the system is observed to be free of monitoring alarms. <br /> Page 1 of 2 <br />
