Charter Way Program: UST <br /> #2 Secondary containment testing is due once every 36 months. The last test was performed <br /> on 10-16-02 and was due on 10-16-05. This testing is past due. Immediately schedule this <br /> testing and notify this office at least 48 hours prior to testing. <br /> #6 Monitoring and response plans are required to be kept onsite. A copy of these plans <br /> could not be located at the time of inspection. Immediately provide a copy onsite. <br /> #7 Maintenance and monitoring records are required to be kept onsite. These records could <br /> not be located at the time of inspection. Immediately provide copies onsite. <br /> #10, 12 The presence of a functional UST monitoring system could not be verified. All <br /> power to the store and dispensers were off at the time of inspection. The UST system <br /> must be monitored at all times. Take all necessary action to restore monitoring to the UST <br /> system immediate) . <br /> #18 Spill containers on both tanks contain liquid. Immediately collect all liquid from the spill <br /> containers, determine if it is hazardous, and dispose of it accordingly. <br /> #23 The designated operator shall perform a monthly inspection of the UST system and <br /> provide the owner with a copy of each monthly report. A copy of each monthly report <br /> shall be retained onsite for at least 12 months. No designated operator reports were <br /> available at the time of inspection. Submit a copy of all designated operator inspection <br /> reports for March 2005 through February 2006 immediately. Retain these reports onsite <br /> immediately. <br /> #37 This UST system has been illegally abandoned. No power is supplied to the facility and <br /> the UST system is not being monitored. <br /> 438 The UST system shall be monitored using the method specified on the permit for the UST <br /> system and records of monitoring, testing, repairing and closure shall be kept in sufficient <br /> detail to enable this office to determine if the UST system is in compliance. The UST <br /> system is not being monitored at all and no records are available for review. <br /> NOTE: Submit a copy of the "Return to Compliance" form with a statement of how each <br /> violation has been or will be corrected.
