EMERGENCY REPSONSE PLAN <br /> UNDERGROUND STORAGE TANK MONITORING PROGRAM <br /> This monitoring program must be kept at the UST location at all times.The information on this monitoring <br /> program are conditions of the operating permit.The permit holder must notify San Joaquin County <br /> Environmental Health Department,phone(209)468-3420 of any changes to the monitoring procedures,unless <br /> required to obtain approval before making the change.Required by CCR 2632(d)and 2641(h). <br /> FACILITY NAME DATE <br /> FACILITY ADDRESS <br /> 1.List the name(s)and title(s) of the people responsible for <br /> 2.If an unauthorized release occurs, how will the hazardous substance be cleaned up? <br /> Note:If a released hazardous substance reaches the environment, enters a storm drain,or <br /> increases the fire/explosion hazard,then San Joaquin County Environmental Health <br /> Department must be notified immediately.If a hazardous substance is not cleaned up from <br /> the secondary containment within 8 hours,or deteriorates the secondary containment,then <br /> San Joaquin County Environmental Health Department must be notified within 24 hours. <br /> Call(209) 468-3420. <br /> 3.
