In <br /> particular, with the sumps being filled and tested with water numerous <br /> times in an effort to identify and stop the sump leaks, see item 15, probe <br /> wiring was affected by the moisture and became difficult to clear. There <br /> is no separate documentation or service work orders recorded for <br /> correcting the alarms because this maintenance was being performed <br /> under a contracted bid amount. <br /> Re: Jan. 1 Ib—This was a false alarm inadvertently set off while testing. <br /> On this date Afforda-Test was scheduled to accomplish annual testing. <br /> However, only the leak detectors and spill buckets were tested and <br /> certified on this day due to a UST monitor printer problem. Afforda-Test <br /> was rescheduled to perform annual monitor certification on 01/30/06. In <br /> the meantime, SSSE Co. was called to repair the printer, however, it was <br /> determined that the printer would have to be replaced. Replacement <br /> printer ordered and replaced by BEI on 01/25/06. Afforda-Test returned <br /> on 01/30/06 and accomplished annual monitor system certification. <br /> However, in the process the tester inadvertently pulled the contact probe <br /> cable loose and all alarm history was lost. FYI: subsequent to your <br /> inspection a failsafe relay was installed thereby precluding the <br /> requirement for an annual line tightness test. See item#27. <br /> Ref item# 15: Secondary containment for UST is tight. <br /> Discrepancies noted during SB989 testing of August 17, 2005, were <br /> handled in two phases. The project was sent out for bid in two phases. <br /> The first phase was to repair leaks found in all four pump sumps. The <br /> second phase was to locate and repair leaks in the diesel and 91 octane <br /> secondary containment lines. In early October Bagley Enterprises, Inc. <br /> (BEI), was hired by Shawn Corporation to manage repairs of the first <br /> phase of work to be done. A purchase order was issued from BEI to Elite <br /> IV Contractors to make the necessary repairs to the four compromised <br /> pump sumps. Significant problems were encountered in the repair of the <br /> sumps, however, repairs were finally made and the job was closed out on <br /> 01/06/2006. On 01/31/2006 BEI issued a second purchase order to Elite <br /> IV for the repair of the secondary lines. At the time of this writing the <br /> repairs have been made, tested, and awaiting final inspection scheduled for <br /> Friday, 02/24/2006, at 10:30 a.m. <br /> Ref item# 16: All releases in secondary piping flow to a collection sump. <br /> All discrepancies noted have been corrected and awaiting final inspection <br /> scheduled for Friday, 02/24/2006, at 10:30 a.m. <br />
