Additionally,the EHD Inspector on scene, Ray <br /> Von Flue, requested that a line tightness test be accomplished due to the apparent failure of <br /> positive shutdown when it was noted that the diesel pump sump sensor was found inoperative. <br /> Reference Item 47: Maintenance and monitoring records: Test results for previous monitoring <br /> system certification and secondary containment tests were located and placed in the USTDO <br /> Notebook that is kept on-site. Store Manager advised of importance of keeping records on-site- <br /> Reference Item#12: UST Monitoring equipment is operational: This item was marked as a <br /> Repeat Violation. The turbine sump sensor for diesel product was not operational. Further <br /> troubleshooting by the tester and service tech found a broken wire in the junction box. The wire <br /> was repaired and the sensor worked normally when retested. A permit was applied for and <br /> approved. It should also be noted here that the failure noted last year was caused by a faulty line <br /> leak detector on the 89 octane line(nothing to do with this years' failure). In short,the problem <br /> this year is not a repeat of the problem that occurred last year. See attached copy of Inspection <br /> Report dated 01/10/2008. <br /> Reference Item#27: Annual line tightness test: This item was marked as a Repeat Violation. <br /> This facility's monitoring system was found to not have sensor out shutdown. The manager on <br /> site opted to have four(4)line tightness tests conducted in order to comply with this requirement <br /> in lieu of not having sensor out shutdown. Further troubleshooting and investigating revealed <br /> that sensor out shutdown capability had changed. Discrepancies corrected on 01/23/2009 and <br /> monitoring system reprogrammed and floats changed to correct position(normally open). (see <br /> Bagley Invoice#4289, DATED 01/31/2009. It should also be noted here that"sensor out <br /> shutdown"has not been noted as a violation in past inspection reports. Particularly,this <br /> discrepancy was not noted the previous year. See attached copy of Inspection Report dated <br /> 01/10/2008. <br />
