San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California95205-6232 <br /> Telephone: (209) 468-3420 Fax: (209) 468-3433 Web:www.siuov.ora/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> MORADA CHEVRON FAST N EASY#60' 1 10878 N HWY 99 STOCKTON <br /> SUMMARY OF VIOLATIONS <br /> ICLASS I,CLASS 11,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the November 2015 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include: An L2: 91-2 STP Sump Fuel Alarm on November 16, 2015. During the monthly inspection, the <br /> designated operator shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> The designated operator failed to check and note the correct test dates for the Monitoring System Certification, Leak <br /> Detectors and Spill container test on the August 2015 through April 2016 and the May 2015 through June 2015 <br /> designated operator monthly inspection reports. The test date listed on the inspection reports is April 21, 2015. The <br /> correct test date on file is April 22, 2015. During the monthly inspection, the designated operator shall check that all <br /> required testing and maintenance for the UST system have been completed and document the dates they were done <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> This is a repeat violation, Class 11. <br /> 319 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 91-2 and 87 STP sumps. If water could enter into the secondary containment by <br /> precipitation or infiltration, it must be removed and disposed of properly. The service technician removed 5 cups of <br /> liquid from the Premium-2 and Regular STP sumps. Ensure that all sumps and annular spaces are maintained free of <br /> liquid. This was corrected on site. <br /> This is a minor violation. <br /> 325 CCR 2635(b) Spill container failed to contain a minimum capacity of five gallons. <br /> The 87.. 91-2, and diesel direct-bury spill containers failed when tested. All spill containers shall have a minimum <br /> capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br /> discontinue deposition of petroleum into this tank until the component is repaired or replaced by a properly licensed, <br /> trained, and certified contractor under permit and inspection of the EHD. The technicians replaced the caps and <br /> retested. All spill containers passed. This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 25, 2016 . <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Page 4 of 5 <br />
