UST Tank Information forms for the 87 and 91 tanks are not current in CERS. The tank forms incorrectly list external audible/visual as an overfill on site and do not list any corrosion protection. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The site plan submitted does not show the locations of the sensors. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 109 CCR 2637 Failed to test secondary containment. Secondary containment testing was last performed on January 11, 2012 and was completed on March 16, 2015, two months past due. Secondary containment testing is required once every 36 months. Take all necessary precautions to ensure testing is performed in a timely manner. This is a Class II violation. 125 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on April 2, 2013 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by May 2, 2013. A deficiency letter was issued on December 11, 2013. This information has not been received resulting in a non-compliant status for this facility. The following violations are outstanding: CCR 2638 -Monitoring equipment certification not performed annually HSC 25291(f)-Annual line leak detector test not performed HSC 25284.2- Failed to test spill containers annually CCR 2751(c)- Designated operator did not inspect all required items and/or the inspection reports not completed An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken.
