Any change of information must be <br /> updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and <br /> submit for review by the EHD. <br /> This is a Class II violation. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted. The site plan submitted does not show the locations of the <br /> sensors. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log <br /> into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST <br /> Monitoring Site Plan. <br /> This is a minor violation. <br /> 109 CCR 2637 Failed to test secondary containment. <br /> Secondary containment testing was last performed on January 11, 2012 and was completed on March 16, 2015, two <br /> months past due. Secondary containment testing is required once every 36 months. Take all necessary precautions <br /> to ensure testing is performed in a timely manner. <br /> This is a Class II violation. <br /> 125 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on April 2, 2013 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by May 2, 2013. A <br /> deficiency letter was issued on December 11, 2013. This information has not been received resulting in a <br /> non-compliant status for this facility. <br /> The following violations are outstanding: <br /> CCR 2638 -Monitoring equipment certification not performed annually <br /> HSC 25291(f)-Annual line leak detector test not performed <br /> HSC 25284.2- Failed to test spill containers annually <br /> CCR 2751(c)- Designated operator did not inspect all required items and/or the inspection reports not completed <br /> An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken.
