Hwy 99 Stockton CA 95215 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> On site to conduct a routine inspection and witness the monitoring system certification. <br /> 11-29-10 ADDENDUM TO OIR COMPLETED AND EMAILED TO BUSINESS OWNER. <br /> 2. Secondary containment testing was performed on 4-10-08 and a test report was submitted to the EHD <br /> 10-22-08, five months late. A copy of the test report must be submitted within 30 days of the test. <br /> Continue to ensure that a copy of the test report to the EHD on time. <br /> 5 & 6. Current approved copies of the response plan were not found on site. The response plan must be <br /> current and approved by the EHD. Complete and submit a copy of the response plan for approval by the <br /> EHD by 12-24-10. An approved copy will be returned for your records. <br /> 7. Maintenance records for the C1 87 fill sump 2-2-10, 2-21-10, 3-1-10, 6-9-10, 6-17-10, and 6-18-10 <br /> fuel alarms; 91 fill sump 2-27-10 fuel alarm; 91 stp sump 4-1-10, 4-19-10, 4-22-10, 4-23-10, 4-24-10, <br /> 6-14-10, 6-28-10, and 6-29-10 sensor out alarms were not found on site. These records shall be kept on <br /> site for at least 3 years. Immediately locate and maintain the Maintenance records for the C1 87 fill <br /> sump 2-2-10, 2-21-10, 3-1-10, 6-9-10, 6-17-10, and 6-18-10 fuel alarms; 91 fill sump 2-27-10 fuel alarm; <br /> 91 stp sump 4-1-10, 4-19-10, 4-22-10, 4-23-10, 4-24-10, 6-14-10, 6-28-10, and 6-29-10 sensor out <br /> alarms on site and submit copies to the EHD by 12-24-10. Repeat Violation. <br /> 17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br /> of detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak detector, <br /> retested it, and it passed. <br /> 21. Liquid was observed in the 89 and 91 stp sumps. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br /> remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br /> and manage it accordingly. Ensure that the 89 and 91 stp sumps are maintained free of liquid. Repeat <br /> Violation. <br /> 23c. The designated operator failed to attach the alarm history print out for the previous month for the <br /> 2-18-10 designated operator report. The designated operator failed to attach the alarm history print out <br /> for the 87 fill sump for the previous month for the 1-21-10 designated operator report. The designated <br /> operator failed to attach the alarm history print out for the previous month for the 9-30-10 designated <br /> operator report. The designated operator failed to document all the alarms for the previous month and <br /> check that they were responded to appropriately on the 4-14-10 designated operator monthly inspection <br /> report.
