The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Stacy Rivera at (209) 468-3440 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Secondary containment testing on the slop tank sump <br /> 4. If concrete will be broken near the UST or piping, a secondary containment test must be <br /> performed on the affected areas once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 5. Secondary containment testing for the slop tank sump is required six months after <br /> installation. Provide 48 hours notification to the EHD when this test is scheduled. <br /> 6. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible <br /> valid, current contractor's licenses issued by the Contractors State License Board <br /> (may be issued to the installation company), or a Tank Tester's license issued to the <br /> individual by the State Water Board, and <br />
