Pershing Ave <br /> Stockton, CA 95207 ENVIRONMENTAL HEALTH <br /> Inspection Report DEPARTMENT <br /> Dear Mr. Ruvalcaba: <br /> The intent of this letter is to respond to the item(s)you noted on your Inspection Report issued to <br /> the location referenced above dated 10/3/2017. <br /> Item 1: On your inspection report you noted a few changes needed to be made to the <br /> information entered into the states CERS system for this store. Specifically, you noted the tank <br /> forms needed to be updated to show no audible of visual alarms and the monitoring and site <br /> plans needed to reflect a different sensor model numbers for the tank 1 piping and UDC's. <br /> Resolution 1: The requested updates noted were completed 10/6/2017 and are available for <br /> review in CERS. <br /> Item 2: On your inspection report you noted that an updated copy of this stores monitoring plan <br /> needed to be retained on site. <br /> Resolution 2: A copy of the stores updated monitoring plans, reflecting the changes noted <br /> above, were placed were placed in the stores records by the assigned Designated Operator on <br /> 10/30/2017. Copies are also available for review in CERS. <br /> Item 3: On your inspection report you noted that Juan Valles had not been trained by the <br /> Designated Operator. <br /> Resolution 3: Juan received training by the Designated Operator on 10/30/2017.
