)r Yellow Freight <br />(SB 989 Repairs) <br />1535 E. Pescadero Way, Tracy, Ca <br />Results of the failed SB 989 Testing are attached for your review. The intent of this submittal is to <br />address the failures by the application of Blue Late Repair fittings. <br />Blue Line Split Repair Entry fittings shall be utilized to repair the existing failed entry or <br />penetration fittings as delineated on the test results. In some cases, and in addition to the failure of <br />a `wall penetration" or "entry" fitting failure, the fiberglass attachment to the tank collar requires <br />repairs. It is also the intent, if necessary, to apply 3 layers of fiberglass matting with Isophalyic <br />resins to this sump/collar connection. (It is understood that only the Tank Manufacturer make <br />FRP repairs to the tank shell). <br />The secondary lines however shall require further investigation. It is the intent if this submittal to <br />provide and install the necessary secondary test boots and to perform locating only. Once the <br />underground issue are itemized and locate then the SJCEHD shall be provided an addendum <br />detailing these phase 2 repairs. <br />Upon successful completion of the above the site will then be ready for inspection. <br />• San Joaquin County Inspector shall be provided a minimum of 72 hours notice for any <br />requested inspection. All inspection times are the discretion of the San Joaquin County <br />Health Dept. <br />• A Blue -Line certified individual will perform all repairs, no exceptions. Copies of Tank- <br />Tek's Blue -Line cent's, and ICC are attached with this submittal <br />• All personnel on this project shall possess a minimum of 40 Hour OSHA course or the 8 <br />Hour OSHA refresher course. Individuals unable to demonstrate this training will not be <br />allowed on the premises. <br />• A copy of the SB 989 Certification shall be forward to the San Joaquin County Health <br />Dept within 30 days of the test. <br />
