Alarm sensor log was not found on site. Any and all alarms must be recorded by employees if they <br /> were not documented by the designated operator. No documentation on alarm logs or monthly reports <br /> can be found for the above alarms referenced in violation #204. These records shall be kept on site for <br /> at least 3 years. Immediately locate maintenance records/work orders and maintain the alarm sensor <br /> logs on site. Work orders should be attached to the designated operator's monthly reports. Submit <br /> copies of these maintenance records to EHD associated with the above alarms. <br /> 402. Primary containment not liquid tight. <br /> -Approximately half an inch of liquid was observed in the Auto Side vent box but insufficient amount to <br /> trigger an alarm. Liquid was removed by technician. <br /> -Approximately 3/4 cup of diesel was observed in the Auto Side Diesel STP sump. Per technician, when <br /> he removed the diesel liquid detection sensor from the diesel STP sump to test for functionality, diesel <br /> was dripping off of the liquid sensor but was insufficient amount to trigger an alarm. Diesel was removed <br /> with a clean white hydrocarbon absorbent pad which became saturated dark and oily with diesel. <br /> -Approximately 3/4 gallon of diesel was observed in the Truck Side North Diesel STP sump. Liquid <br /> detection sensor was located opposite side of the spillage of the concave sump top. Diesel was placed <br /> back into the tank. <br /> -Approximately <1/2 gallon of diesel was observed in the Truck Side South Diesel STP sump. Liquid <br /> detection sensor was located opposite side of the spillage of the concave sump top. Diesel was placed <br /> back into the tank. <br /> -Approximately less than 1/8 inch of diesel can be observed in all Truck Side Diesel dispensers. Amount <br /> insufficient to trigger an alarm. <br /> All primary containment must be impervious to the liquid and vapor of the substance contained. <br /> Immediately discontinue use of failed piping and have a properly licensed, trained and certified <br /> contractor replaced or repair the failed piping under permit and inspection of EHD. If the failed <br /> components can't be replaced/ repaired immediately, there is a possibility that the Auto & Truck Side <br /> diesel UST system may be red tagged to prevent fuel inputs. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Receiv By: Title: <br /> Thuy Tran (209) 468-8257 f <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
