1. The permit to operate is being held by the EHD pending the payment of some outstanding <br /> invoices.The EHD will continue to hold the permit until all invoices are paid. Unfortunately the <br /> EHD has continually added new invoices for hours and fees,which has made it difficult to stay <br /> ahead of the billings.We are currently working to resolve this issue so that we can be sent to <br /> permit. <br /> 6. Please see the answer above. <br /> 19.The light was fixed on site.The audible sound was working at the time of repair. <br /> 23.Letter sent to DO regarding issues noted on the inspection report. DO was also informed in <br /> person that failure to report alarms would result in a termination of the current DO company from <br /> this and our other sites. <br /> 7.Alarm log has been posted in the site binders for updating should alarms occur on site.DO is cross <br /> checking this log to verify corrective action taken. <br /> 32.After speaking with Able Maintenance regarding the replacement of a float switch,it was <br /> discovered that a permit was not applied for at that time.Marty with Able/SSS contacted the EHD in <br /> order to clear up the issue.According to Able/SSS the paperwork has been submitted and is/was <br /> pending approval. <br /> 43.on 5/23/11 Service Station Systems sent out a technician to investigate the leak in the STP <br /> Sumps for the North and South Diesel tanks. It was determined that there was a slow leak from the <br /> piping that did not escape the primary or secondary containment.On 6/13/11 Able returned to the <br /> site for further investigation and repair if possible.At that time they were able to complete the <br /> repair.The work order is attached to this statement. <br /> 22.After investigation by Service Station Systems and myself on 5/23/11 it was determined that <br /> they leak in both UDC's was caused by the loose connection between the underground piping the <br /> dispenser piping.All connections were tightened on 5/23/11. Frequent monitoring has shown no <br /> signs of the leaks reoccurrence in either UDC.We will continue to monitor the UDC's with visual <br /> inspections apart from the monthly DO visual inspections.Should a leak be spotted we will submit a <br /> report and repair as soon as possible.A report for the release is attached to this statement. <br /> 23d.Training was scheduled for May 23,2011 and was completed at that time.All employees are in <br /> compliance with DO training requirements.New employees will be trained within 30 day and <br /> continuing employees will be retrained in May 2012. <br />
