A payment plan was worked out with Accounting Department of the San Joaquin County <br /> Environmental Health Department.The first payment was made to the EHD in November. <br /> 2. Financial responsibility forms sent to site and submitted to EHD with this form. <br /> 3. UST Monitoring plan and response plan submitted with this form. Forms also sent to site. <br /> 4. The UST operating permit will be sent on completion of the payment plan to the EHD.However, <br /> we are able to continue operating legally as we have a payment plan to clear up the past due <br /> balance. <br /> 5. Monitoring certification, line leak detector,and spill bucket tests sent to site and delivered to <br /> EHD by site manager. <br /> 6. Diesel north fill sump sensor permit given and replaced by Service Station Systems(work <br /> ordered attached). <br /> 7. Release form attached. <br /> 8. Release form attached. Exact cause of the leak into the dispenser pans is not known at this time. <br /> Site will monitor the pans and sumps to see if further issues occur or a detectable leak presents <br /> itself. Final certification of sites leak sensors on December 8,2010 will ensure they can properly <br /> detect release. <br /> 9. In order to help the Designated.Operator report more completely each month the site will <br /> maintain a written log at the monitoring panel. On site manager will follow up to ensure DO has <br /> properly address the noted alarms during their monthly inspection. <br /> 10. Service Station System called to investigate the cause of the liquid in both the diesel STP sump <br /> and in the vent box. Initial investigation showed no cause of the liquid thus a more thorough <br /> investigation into a possible issue. <br />
