The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> Individuals installing UST Systems or components shall possess or work under the direct and <br /> personal supervision of an individual physically present at the work site who possesses all of <br /> the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> b. A valid, current International Code Council (ICC) UST Installer/Retrofitting certificate <br /> and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being installed. <br /> Qualified service technicians shall possessor work under the direct and personal supervision <br /> of an individual physically present at the work site who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the component(s) <br /> being tested. <br /> Each individual installing or testing any component shall hold a certificate of training from the <br /> manufacturer of that component. <br /> 5 Any changes shall be submitted to the Environmental Health Department(EHD)for approval <br /> prior to commence work. All new submittals supercede the previous ones. <br /> 6 All piping, fittings, sumps, and under-dispenser containment must be certified by an <br /> independent testing organization and labeled as required by the organization. (UL, ULC, ANSI, <br /> API, ASTM, etc. See California Code of Regulations, Title 23, Chapter 16 Table B for <br /> complete list.). Pursuant to the Underwriters Laboratories Inc(UL) listings, only those <br /> products bearing the UL mark will be considered listed (only applicable to UL listed products). <br /> 7 All monitoring systems shall be listed on the LG113 (current version). <br /> e• <br /> Page 1 of 2 <br />
