No. TBD / Our Job No. 16700 <br />On behalf of our client Ken Dharni, Barghausen Consulting Engineers, Inc. is submitting to the <br />Environmental Health Department (EHD) the required documents for the project referenced above. The <br />scope of work is a new ARCO AM/PM of 3,180 square feet, an eight (8) multi-product dispenser (MPD), a <br />50- by 121-foot canopy, a 26- by 60-foot car wash, and two underground storage tanks. <br />Enclosed for your review are the following documents: <br />1. One (1) completed San Joaquin County EHD Service Request Form and Fee Sheet <br />2. One (1) completed Underground Storage Tank System Installation Permit Application <br />3. One (1) completed UPCF Underground Storage Tank — Facility Form A <br />4. One (1) completed UPCF Underground Storage Tank — Tank Form B per tank <br />5. One (1) set of manufacturer's Equipment Cut Sheets <br />6. Three (3) full-sized sets of plans including specifications and buoyancy calculations <br />7.
