Designated operator monthly inspection reports for the <br />previous twelve months shall be retained on site. Ensure that copies of the previous twelve months of designated <br />operator monthly inspection reports are maintained on site. The Designated Operator faxed over the reports and <br />they were printed out. <br />This was corrected on site. <br />This is a minor violation. <br /> 211 CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The hiring dates for all of the employees were absent from the training sheet. <br /> The designated operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility’s best management practices <br /> 2. Employee’s role with regard to monitoring equipment as specified in the facility’s monitoring plan <br /> 3. Employee’s role with regard to spills and overfills as specified in the facility’s response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that the hiring dates are included on the training forms for future training sessions. <br />This was corrected on site. <br />This is a minor violation. <br /> 309 CCR 2636(f)(1) Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. <br />The 91 grade Bravo box float and chain for the 3/4 UDC sump failed to stop the flow of product at the dispenser <br />when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow <br />of product at the dispenser when it detects a leak.
