Underground Storage Tank Program Inspection Report <br />Date: <br />September 05, 2017 <br /> Facility Address: <br /> 20 W TURNER RD, LODI <br /> Facility Name: <br /> SHORT STOP FOOD MART <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 210 CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />The December 2016, January 2017, February 2017, March 2017, April 2017, May 2017, and June 2017 designated <br />operator monthly inspection reports were not found on site. Designated operator monthly inspection reports for the <br />previous twelve months shall be retained on site. Ensure that copies of the previous twelve months of designated <br />operator monthly inspection reports are maintained on site. The Designated Operator faxed over the reports and <br />they were printed out. <br />This was corrected on site. <br />This is a minor violation. <br /> 211 CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The hiring dates for all of the employees were absent from the training sheet. <br /> The designated operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility’s best management practices <br /> 2. Employee’s role with regard to monitoring equipment as specified in the facility’s monitoring plan <br /> 3. Employee’s role with regard to spills and overfills as specified in the facility’s response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that the hiring dates are included on the training forms for future training sessions. <br />This was corrected on site. <br />This is a minor violation. <br /> 309 CCR 2636(f)(1) Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. <br />The 91 grade Bravo box float and chain for the 3/4 UDC sump failed to stop the flow of product at the dispenser <br />when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow <br />of product at the dispenser when it detects a leak. The service technician adjusted the chain and verified <br />functionality. <br />This was corrected on site. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by October 4, 2017 . <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />Page 5 of 6 <br />FA0000645 PR0231380 SC001 09/05/2017 <br />EHD 23-01 Rev. 09/22/16 Underground Storage Tank Program OIR
