UST Monitoring Plan • • <br /> Stockton Service Center <br /> A leak will be confirmed as a result of investigation and test findings of the service representative <br /> as to nature, extent, and location of the leak source, i.e., inner tank or outer tank. Contact the <br /> Division Environmental Specialist and take the following actions, as applicable. <br /> a. Discontinue tank use. <br /> b. Contact disposal agency to remove contents from the tank: <br /> Evergreen Environmental Services <br /> 6880 Smith Avenue <br /> Newark, CA 94560 <br /> (415) 795-4410 <br /> 1-800-972-5284 <br /> C. Notify local permitting agency immediately by telephone, and inform them of the situation <br /> and actions taken. Follow up with a letter within 5 days (See VII-E below). <br /> d. Coordinate with the Division Environmental Specialist to schedule preliminary <br /> investigation of the tank to determine the extent of the damage and what remedial action <br /> should betaken, i.e., repair or closure. <br /> e. Arrange for an alternate waste oil storage to meet operation needs. i.e., at another <br /> company facility, an off-site commercial service station, install an above ground portable <br /> storage tank, etc. Coordinate arrangements with the Division Environmental Specialist to <br /> insure compliance with jurisdictional regulatory agencies. <br /> VII. PROGRAM ADMINISTRATION AND MAINTENANCE: <br /> The Division Fleet Supervisor is responsible for insuring that the Underground Storage Tank Compliance <br /> Program is administered effectively, that facility records are maintained properly, and are accessible for <br /> review or audit by authorized agencies. The following are the minimum controls that must be implemented: <br /> A. All system circuit checks, alarm conditions, and service calls must be documented. Records will be <br /> maintained at the facility for a period of 3 years. <br /> B. The system shall be inspected monthly by the Designated UST Operator. The inspection will <br /> document on the prescribed log form. The form will be kept at the facility. The Designated Operator <br /> must be certified in the State of California as a UST Designated Operator. <br /> C. Employees responsible for tank operation/leak response must be familiar with, and understand the <br /> contents of this manual. <br /> D. Employees responsible for the tank operation/leak response must be familiar with the operation, <br /> maintenance and service requirements of the tank monitoring systems. <br /> 1. Monitoring devices are serviced annually to maintain equipment accuracy and reliability. <br /> 2. If the monitoring devices malfunction, fail to operate, or otherwise operate abnormally; the <br /> monitoring service representative must be called to initiate remedial action. <br /> E.
