EMERGENCY PROCEDURES <br /> On-site personnel will use the following standard emergency procedures. The SSHO <br /> shall be notified of any on-site emergencies and be responsible for ensuring that the <br /> appropriate procedures are followed. <br /> PERSONNEL INJURY: <br /> Upon notification of an injury, the SSHO will assess the nature of the injury. If the cause <br /> of the injury does not affect the performance of site personnel, operations may continue, <br /> with the appropriate on-site field personnel initiating the appropriate First-Aid and <br /> necessary follow-up. If the injury increases the risk to others, all field personnel shall be <br /> moved to a safe location until further instructions. <br /> Activities on site will stop until the added risk is removed or minimized. <br /> FIRE/EXPLOSION: <br /> In the event of a fire or explosion on site, the Fire Department well be summoned. All <br /> site personnel shall assemble at "rally point" a safe distance from the involved area. <br /> PERSONAL PROTECTIVE EQUIPMENT FAILURE: <br /> If any site worker experiences a failure or alteration of protective equipment that affects <br /> the protection factor, that person shall immediately stop that particular work activity and <br /> repair or replace the failure. Should the failure of the personal protection equipment be <br /> beyond repair the worker will stop that work activity and report the failure to the SSHO. <br /> That site worker will be reassigned to other work activity not requiring that particular <br /> PPE and shall not be permitted to allowed to continue the previous work activity until the <br /> equipment has been repaired or replaced. <br /> EMERGENCY RESPONSE FOLLOW-UP <br /> The SSHO shall submit an Incident Report to the Project Manager. This Report shall <br /> include the following: <br /> • Chronological history of the incident; <br /> • Facts about the incident and when they became available; <br /> • Titles and names of personnel; <br /> • Actions: Decisions made and by whom; Order given to whom, and by <br /> whom; and when; Action taken, who did what, when, where, and how; <br /> • Possible exposure of site personnel; J U L 14 2017 <br /> • Report all injuries or illnesses during or as a result of the emergency. <br />
