The permit has <br /> been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Michelle Henry at (209) 468-3454 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. EHD must witness the functionality of the monitoring system to ensure that <br /> monitoring has been restored to meet the requirements of California Code of <br /> Regulations Title 23 and the Health & Safety Code. <br /> b. Secondary piping soap test: One hour pressure test per manufacturer's guidelines. If <br /> there are no manufacturer's guidelines, test shall be performed using an applicable <br /> method specified in an industry code or engineering standard. <br /> 4. Secondary containment testing for the aboveground secondary containment pipe on the <br /> waste oil tank fill line is required six months after installation.
