A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. This is a repeat violation, Class II. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment testing on the March 2018 designated operator monthly inspection report. This testing was conducted on 2/8/18 and the new test date was correctly noted on the February 2018 inspection report. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 318 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. The 87 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. This was corrected on site. This is a repeat violation, Class II. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by July 14, 2018. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written Agreement, if applicable, can be stored at the facility or uploaded into CERS.
