San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax: (209)468-3433 Web:www sioov.or /q ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> MY MINI MART 1756 N WILSON WAY STOCKTON June 29 2017 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 309 CCR 2636(f)(1) Monitoring in sump/ UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The south, 87, Bravo float and chain in the dispenser 1/2 under dispenser containment sump failed to stop the flow of <br /> product at the dispenser when tested. The float probably has a hole in it. All monitoring equipment shall be <br /> maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. <br /> Correct immediately by having a properly licensed, trained, and certified contractor replace the failed component with a <br /> functional component(LG 113-x listed, if applicable). Provide notification to the EHD with the following information for <br /> like-for-like replacements: Technician company name, ICC service technician certification, Veeder Root certification, <br /> brand and model number of the new sensor, time and date of sensor installation. Provide test results within 30 days <br /> of the repair. Apply for a repair permit with the EHD within one business day for replacements that are not like-for-like. <br /> If the failed component can't be replaced immediately, there is a possibility that the 87 octane UST system may be <br /> red tagged to prevent fuel inputs. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> John Alaniz started the SJCEHD inspection and witnessed the liquid sensor testing, sensor out/fail safe and <br /> Bravo float and chain testing. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by July 29, 2017 . <br /> AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING THIS <br /> FACILITY BACK INTO COMPLIANCE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Received by: Printed Name and Title:Date: G /1 ,1 / (7 Inspector: Inspector Phone: <br /> S Uj,;WIM �' �"A l�U�i�') GARRETT BACKUS, Lead Senior REHS (209)468-2986 <br /> / <br /> .vl'I,Qy11 W_ "/V-�wii IV,I, Page 4 of 4 <br />
