San Joaquin County <br /> Environmental Health Department <br /> h <j <br /> Complaint Record Daily Time and Activity Summary from 3/1/2015 to <br /> 6/21/2034 for C00042165 located at <br /> 1756 N WILSON WAY <br /> Activity Date Recorded by <br /> 08/31/2016 ER Response - EHD left department 10:12 arrived on site 10:24. EHD observed PE 2546 MEDINA <br /> iso min area(s)at Pump#4 to have remanants in several locations, pictures taken. EHD SC 544 <br /> spoke with on site employee, Dhani Sachan who stated he did not know anything <br /> abouth the spill. EHD spoke with off-site manager, Vinay Verm who stated he did <br /> not know anything about the spill. EHD asked where the location of the spill <br /> contentst was located and EHD was directed to storage area in facility to a <br /> Tice-gallon lableed bucket. Contents of bucket included bentonite granules and a <br /> gasoline odor was detected from the waste bucket. EHD directed manager to <br /> clean-up the remaining absorbent left at the spill areas and to remove contents of <br /> the bucket to the HHW facility(brochure provided by EHD) and provide receipt of <br /> disposal to the EHD within 30 days. <br /> Notice To Abate processed on site at 1756 N Wilson Way, Stockton. <br /> EHD drove to meet off-site manager, Vinay Verm located at another facility off <br /> Fremont St, Stockton, CA. Mr. Verm stated he could not leave the facility to come <br /> to the affected site since he was the only one staffed at this location. <br /> Arrived back to EHD 12,55 pm and processed Prop 65. <br /> 08/31/2016 Prop 65 generated, printed and submitted to G. Backus for approval and PE 2546 MEDINA <br /> 24 min signature. SC 203 <br /> 09/06/2016 Called Joginder Lal (owner) 775 421-9978 to inquire if he was able to clean-up PE 2546 MEDINA <br /> 12 min and transport spill contents from 8/31/2016 to the HHW with a receipt to be SC 544 <br /> provided to the EHD.Mr. Lal stated he would check with his manager(Vinay Verm) <br /> to see if he could drop it by. EHD stated the receipt could be faxed to 209 <br /> 468-3433 or emailed directly to Imedina@sjcehd.com. EHD stated compliance <br /> date is 9/3012016. <br /> 09/15/2016 EHD received confirmaton receipt of waste disposal from 8/31/2016 Spill Report PE 2546 MEDINA <br /> 12 Rin Complaint abated this date. SC 544 <br /> Page 1 of 1 <br />
