Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Date: <br />May 30, 2018 <br /> Facility Address: <br /> 14931 N FLAG CITY BLVD, LODI <br /> Facility Name: <br /> LODI OIL INC ARCO #83680 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br />The monitoring panel activated a visual alarm, but failed to activate an audible alarm when tested to ensure <br />continuous monitoring of the tank system. According to the service technician, Chris Bishop and manager, Veer <br />Singh, the electronic sound horn was disconnected from the monitoring panel. According to both Chris Bishop <br />(during air quality testing) and Veer Singh, it was noticed about a month and a half ago. All monitoring equipment <br />shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it <br />detects a leak. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed <br />component with a functional component (LG 113-x listed, if applicable) and obtain a permit within one business day <br />from the EHD. <br />This is a Class II violation. <br /> 315 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br />Liquid was observed in the 87-product submersible turbine pump (STP) sump, the diesel STP sump, and the under <br />dispenser containment 9/10, and 3/4. If water could enter into the secondary containment by precipitation or <br />infiltration, it must be removed and disposed of properly. The service technician removed about 3 cups of liquid <br />from the 87/diesel STP sumps, 1/4 gallon of liquid from UDC 3/4, and 3 gallons of liquid from UDC 9/10. Ensure that <br />all sumps and annular spaces are maintained free of liquid. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />Onsite for routine and monitoring system certification. Did not witness inside 87-product, 91-product, and diesel <br />product fill sumps. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receipt of this report. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br />applicable), and the Owner Statement of Designated Operator Compliance.
