The following is an itemized list of underground storage tank violations that have not <br />been addressed for LODI OIL INC ARCO #83680 as of July 13, 2018. <br />Open violations from May 30, 2018 inspection <br />Violation #111 - Failed to test monitoring and leak detection equipment annually. <br />The Veeder root 208 sensors in under dispenser containment (UDC) 1/2, 3/4, 5/6, and 9/10 were observed with <br />splicing outside of the junction box. The junction box caps were missing, and spliced wiring were exposed outside of <br />the explosion proof junction box. All monitoring equipment shall be installed, calibrated, operated and maintained in <br />accordance with manufacturer's instructions. Immediately replace junction box caps, and properly install according <br />to manufacturer's recommendations. Obtain a repair permit if necessary. <br />Violation #308 - Failed to maintain a continuous monitoring system that activates an audible and visual <br />alarm. <br />The monitoring panel activated a visual alarm, but failed to activate an audible alarm when tested to ensure <br />continuous monitoring of the tank system. According to the service technician, Chris Bishop and manager, Veer <br />Singh, the electronic sound horn was disconnected from the monitoring panel. According to both Chris Bishop <br />(during air quality testing) and Veer Singh, it was noticed about a month and a half ago. All monitoring equipment <br />shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it <br />detects a leak. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed <br />component with a functional component (LG 113-x listed, if applicable) and obtain a permit within one business day <br />from the EHD. <br />Page 1 of 1 <br />
