The following is an itemized list of underground storage tank violations that have not
been addressed for LODI OIL INC ARCO #83680 as of July 13, 2018.
Open violations from May 30, 2018 inspection
Violation #111 - Failed to test monitoring and leak detection equipment annually.
The Veeder root 208 sensors in under dispenser containment (UDC) 1/2, 3/4, 5/6, and 9/10 were observed with
splicing outside of the junction box. The junction box caps were missing, and spliced wiring were exposed outside of
the explosion proof junction box. All monitoring equipment shall be installed, calibrated, operated and maintained in
accordance with manufacturer's instructions. Immediately replace junction box caps, and properly install according
to manufacturer's recommendations. Obtain a repair permit if necessary.
Violation #308 - Failed to maintain a continuous monitoring system that activates an audible and visual
alarm.
The monitoring panel activated a visual alarm, but failed to activate an audible alarm when tested to ensure
continuous monitoring of the tank system. According to the service technician, Chris Bishop and manager, Veer
Singh, the electronic sound horn was disconnected from the monitoring panel. According to both Chris Bishop
(during air quality testing) and Veer Singh, it was noticed about a month and a half ago. All monitoring equipment
shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it
detects a leak. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed
component with a functional component (LG 113-x listed, if applicable) and obtain a permit within one business day
from the EHD.
