Contact the local fire department Local Planning or Building Department, and the <br /> San Joaquin Valley Air Pollution Control District for any applicable permits and <br /> requirements. <br /> 2. Contact Michelle Henry at(209) 953-7699 to arrange the following inspection <br /> appointment(a 48-hour notice is required for inspection appointments): <br /> a. Functionality test to ensure that monitoring has been restored to meet the <br /> requirements of California Code of Regulations Title 23 and the Health& <br /> Safety Code. <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. If concrete is broken near the UST or product lines, a tank integrity and/or line <br /> integrity test (or equivalent)must be performed, once concrete is poured in <br /> accordance with the manufacturer's guidelines. Results must be received by EHD <br /> within seven days of returning the facility to operating condition. <br /> 5. Qualified installers and service technicians must possess or work under the direct <br /> and personal supervision of an individual physically present at the work site, at all <br /> times while work is being performed, who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. All valid, current California UST certificates issued by the International <br /> Code Council (ICC), that are applicable to the work being performed, and <br />
