One (1) original and two (2) copies of the Application for UST Retrofit or Piping Repair Permit <br /> Form EH230038 <br /> 2. One(1)set of contractor's licensing information and certifications <br /> Please note that the following scope of work is a correction to that listed in our letter dated June 20,2008: <br /> 1. Install a Healy Clean Air Separator(CAS). <br /> 2. Install In-Station Diagnostics(ISD). <br /> 3. Install anew Veeder-Root PLLD at each of the three(3)turbines. <br /> It is our understanding that the UST Retrofit Application will be approved upon receipt of the above <br /> information and enclosed documents.
