No. 658 /Our Job No. 10050.5 <br /> Dear Linda: <br /> On behalf of our client, Costco Wholesale, Barghausen Consulting Engineers, Inc., respectfully requests <br /> that the San Joaquin County Environmental Health Department review and approve the Service request <br /> and Underground Storage Tank Retrofit Application for the Enhanced Vapor Recovery (EVR) Phase 11 <br /> equipment upgrade at the facility referenced above. <br /> Please be aware that the installation of the Healy Clean Air Separator(CAS) is mandated by the State of <br /> California Air Resources Board (CARB) and the San Joaquin Valley Air Pollution Control District <br /> (SJVAPCD). The CARB and the SJVAPCD require that this equipment be installed and operational by <br /> 2009. Costco is submitting this application at this time to ensure that there is sufficient time to complete <br /> the installation prior to the CARB-imposed deadline. <br /> Enclosed for your review and approval are the following: <br /> 1. One(1)original signed and one(1)copy of the Service Request Form EHD 48-02-025 <br /> 2. One(1)original signed and one(1)copy of the Application for UST Retrofit or Piping Repair <br /> Permit Form EH230038 and Form EH23 032 <br /> 3. Two(2) sets of wet-signed plans <br /> 4. One(1)set of manufacturer's equipment cut sheets <br /> 5. One(1) Request for After Hours Certification <br /> 6. One (1) Barghausen Consulting Engineers, Inc. check in the amount of$294, made payable <br /> to San Joaquin Environmental Health Department for the plan check review fees <br /> The scope of this installation includes the following: <br /> 1.
