The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code of Regulations and the Health & Safety Code. <br /> b. If it is determined that a cold start is necessary, a copy of the alarm history shall be <br /> printed prior to performing the "cold start' of the Veeder Root system and made <br /> available at the time of EHD inspection. <br /> c. Leak detector test: Leak detector must be tested to confirm that the device can <br /> detect a leak of 3 gallons per hour at 10 psi. <br /> d. Overfill prevention verification: Verify that external audio and visual alarm activates <br /> when tank is 90% full. <br /> Please be aware that the facility cannot operate the UST tank prior to the EHD <br /> inspection and approval. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br />
