Harding Way <br /> Stockton, CA 95204 <br /> The San Joaquin County Environmental Health Department has been informed that you <br /> are the new owner or operator of the underground storage tanks (UST's) at the above- <br /> mentioned facility. Please complete the following items immediately: <br /> 1. Underground Storage Tanks — Facility Form — Provide all current information, <br /> sign and date at the bottom. <br /> 2. Underground Storage Tanks—Tank Forms—Provide all current information, sign <br /> and date the bottom of each form. <br /> 3. Certification of Financial Responsibility/Letter from Chief Financial Officer: <br /> Complete the form(s) provided, and make a copy for our records. This document <br /> is required to be submitted to this office annually. <br /> 4. Underground Storage Tank Monitoring and Response Plans: Complete and <br /> submit a copy. Indicate in Section IX of the Monitoring Plan the type of overfill <br /> prevention present for each tank. Refer to the enclosed LG 150-1 letter. Retain <br /> the originals onsite for future reference by employees. <br /> 5. Report #5021: Update all information for all programs at this site including the <br /> UST and Hazardous Waste programs, sign and date. Bring with all other forms to <br /> our accounting office to pay for transfer fees and the $95/hr UST Consulting fee, <br /> if necessary. <br /> 6. If you are the owner, you must submit a notification form identifying the <br /> "Designated UST Operator" for each facility owned. A notification form is <br /> enclosed for your use. <br /> 7. Go to: hLtp://www.waterboards.ca.gov/mvhome/ust/regulatory/index.htrnl to <br /> obtain the current statutes (Underground Storage of Hazardous Substances) and <br /> UST Regulations that you are required to have for your monitoring and response <br /> plan. <br />
