Underground Storage Tank Program Inspection Report <br />Date: <br />October 06, 2017 <br /> Facility Address: <br /> 1800 N CALIFORNIA ST, STOCKTON <br /> Facility Name: <br /> ST JOSEPHS HOSPITAL <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. <br />A monitoring system certification, leak detector testing, and spill container testing were performed on 10/21/2016. A <br />test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). A copy <br />of the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. <br />This is a Class II violation. <br /> 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br />An in office inspection report was issued on 3/11/17 identifying information to be submitted to bring this site into <br />compliance. This information was required to be submitted by 4/10/17, a partial return to compliance was received <br />on 3/20/2017. However, a valid BOE was not submitted at that time. This information has not been received <br />resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days <br />to submit a written response that includes a statement documenting corrective actions taken or proposing corrective <br />actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is <br />submitted within 30 days of receiving an inspection report. <br />This is a Class II violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plan were not available for inspection. A copy of these plans shall be <br />accessible on site at all times. Immediately locate and retain a copy on site. <br />This was corrected on site. <br />This is a Class II violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to check and note the correct test dates for the Secondary Containment Testing on <br />the October 2016 to October 2017 designated operator monthly inspection reports. The Designated Operator noted <br />the last Secondary Containment Testing was done on 10/26/2017, but EHD received notification for testing date on <br />10/27/2016. During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required information on <br />the reports. <br />This is a repeat violation, Class II. <br /> 315 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br />Liquid was observed in the diesel sump. If water could enter into the secondary containment by precipitation or <br />infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste <br />determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the diesel sump is <br />maintained free of liquid.
