SAN10 A Q U I N Environmental Health Department <br /> COUNTY <br /> 20. A "Certification of Installation/Modification" form for installed tank systems shall be submitted into <br /> CERS by the owner or the owner's agent, pursuant to CCR 2635(d), before this UST installation will <br /> be considered complete by the EHD. <br /> 21. An accurate and drawn to scale as-built drawing(s), which indicates the location of the UST system <br /> (underground storage tanks, piping, auxiliary and monitoring equipment) with respect to buildings or <br /> other landmarks, per CCR 2711(a)(8), and copies of any required installation checklists (tank <br /> installation, etc.) must be submitted to the EHD prior to scheduling a final inspection. <br /> 22. Designated Operator (DO) training requirement must be satisfied prior to scheduling the final <br /> inspection, per CCR 2715(f). <br /> 23. A valid Board of Equalization (BOE) number must be obtained and provided to EHD prior to fuel drop, <br /> and must be uploaded into the California Environmental Reporting System (CERS) prior to scheduling <br /> the final inspection. <br /> 24. Hazardous material or waste cannot be released, intentionally or unintentionally, to the sewer/septic, <br /> storm water system, or any other waters of the state. If a release occurs it will be considered illegal <br /> hazardous waste disposal and will be subject to enforcement. <br /> 25. Submit to CERS all applicable hazardous materials, hazardous waste and UST submittals. This <br /> submittal must be approved by the EHD prior to scheduling the final inspection. <br /> 26. Passing ELD test results must be received prior to receiving fuel. <br /> 27. All outstanding up-to-date permit fees must be paid prior to receiving the final approval to operate. <br /> 28. This permit expires at the end of the calendar year.
