The Q4 alarms were not <br /> printed on the alarm histories attached to the designated operator monthly inspection reports. During the monthly <br /> inspection, the designated operator shall review the alarm history for the previous month, check that each alarm <br /> was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken <br /> in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections <br /> at this facility are including all of the required information on the reports. This is a repeat violation, Class Il. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> The 89 fill sump sensor failed to activate an audible and visual alarm when tested to ensure continuous monitoring <br /> of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the <br /> flow of product at the dispenser when it detects a leak. The sensor was replaced and retested during the <br /> inspection. A"UST Retrofit Verification with Inspector Already Onsite"form has been completed and provided to the <br /> operator and contractor. This is a minor violation. This was corrected on site. <br /> 310 CCR 2636(f)(1) Sensor in a turbine sump failed to activate an audible and visual alarm or stop the flow product. <br /> The 87 STP sump sensor failed to activate an audible and visual alarm when tested to ensure continuous <br /> monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm <br /> or stop the flow of product at the dispenser when it detects a leak. The sensor was shaken and retested during the <br /> inspection. This is a repeat violation, Class II. This was corrected on site. <br /> Overall Inspection Comments: <br /> Inspection report revised on February 18, 2015. Violation #125 has been added. This inspection report <br /> supercedes all previous reports. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by February 19, 2015. <br /> Consent to perform the inspection, take photos and collect samples was given by Sandra Harbaugh, Fuel <br /> Manager. This facility's designated operator is Maria Guarnelli ([CC expiration date: 4/17/16) . The service <br /> technician was Kristopher Bell (ICC expiration date: 9/11/16), who had the following manufacturer's certifications: <br /> VMI x. 11/18/16, Veeder Root x. 3/4/16. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if <br /> applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator <br /> Compliance. <br /> Note: The wiring 89/91 annular sensor was observed with two splices outside the electrical box.
