Also there was no panel printout of alarm histories for the August 1, <br /> 2016 DO monthly inspection report. During the monthly inspection, the designated operator shall review the alarm <br /> history for the previous month, check that each alarm was documented and responded to appropriately, and attach <br /> a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> This is a repeat violation, Class II. <br /> 302 HSC 25291(b) UST system incapable of detecting leak from primary into secondary containment(pre-Jul 2003). <br /> The 87-product 407 annular sensor failed to activate an audible and visual alarm when tested and, therefore, the <br /> tank is not monitored. The interstitial space shall be monitored with a monitoring system connected to an audible <br /> and visual alarm system. Immediately contact a properly licensed, trained, and certified contractor to replace the <br /> failed component, with a functional component(LG 113-x listed, if applicable), like-for-like within one business day. <br /> Notify the Environmental Health Department a scope of work and time and date of the repair work to <br /> UST@sjcehd.com. The service company must install the part and test the system to verify functionality. The <br /> service company must provide the following documentation to the tank owner/operator for inspector review during <br /> the annual UST inspection: 1. Description of part(s) installed 2. Name and UST ICC number of service technician <br /> doing the work(manufacturer certification proof)and 3. testing done to verify functionality of the repair. <br /> This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The diesel submersible turbine pump (STP)sump 208 sensor was raised approximately one-and-a-half inches off <br /> the lowest point of the sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall <br /> be maintained to be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the <br /> lowest point of the sump by the service technician.
