The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aaron Hang at (209) 468-3453 to arrange the following appointments (a 48-hour notice is <br /> required for inspection appointments). <br /> • Sensor functionality: Functionality of the monitoring system was verified on March 20, 2017 <br /> duringthe annual monitoring system certification. No further testing is needed. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD inspection <br /> and approval. <br /> 4. If concrete will be broken near the diesel#1 tank,a secondary containment test must be performed on <br /> the diesel#1 tank once concrete is poured in accordance with the manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid,current <br /> contractor's licenses issued by the Contractors State License Board (may be issued to the <br /> installation company), or a Tank Tester's license issued to the individual by the State Water <br /> Board,and <br /> b. Possess or work under the direct and personal supervision of an individual physically present at <br /> the work site who possesses a valid,current California UST Service Technician certificate issued <br /> by the International Code Council(ICC),and <br /> c. Possesses a valid,current certificate of training from the manufacturer(s)of the component(s) <br /> being tested,repaired,or serviced. <br />
