SAN:.JOAQUIN Environmental Health Department <br /> COUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> A&A TRANSMISSION 1211 S WILSON WAY, STOCKTON October 31, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> Two blue 55 gallons steel drums containing used oil were observed(full)with an illegible hazardous waste label and <br /> one black sealed drum of used oil with no labeling at all. <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition(contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> informafion. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 30, 2018. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate($152). To schedule an <br /> appointment, please call (209)468-3420. <br /> An emergency coordinator is lacking. There must be at least one emergency coordinator on site or on call to <br /> coordinate emergency response measures, and the following information must be posted by a phone:the name <br /> and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if <br /> present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. <br /> Immediately appoint an emergency coordinator and post the required information by a phone. A form is <br /> provided that can be used for this purpose. This was corrected on site. <br /> Waste streams observed: Waste streams observed, used oil, oily rags, and parts washer rinse. <br /> Inspector provided RTC Certification and Flyer for free CUPA Classes. <br /> AFTER THE COMPLIANCE DATE,EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING <br /> THIS FACILITY BACK INTO COMPLIANCE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Received by: <br /> en Date: October 31,2018 Inspector.
