One 30 gallon black drum of used oil is missing its hazardous property. All hazardous waste <br />containers shall be marked with the following information: <br />1. the words "Hazardous Waste", 2. name and address of generator, 3. hazardous properties, <br />4. physical state, 5. composition (contents), 6. accumulation start date <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all <br />the required information. <br />- Corrected onsite. The hazardous property: toxic was marked <br />30. One 30 gallon black poly drum that formerly held corrosive liquid. Containers larger than five gallons <br />that previously held a hazardous material shall be marked with the date they were emptied and <br />managed within one year of being emptied. Immediately mark and manage all empty containers per this <br />section. <br />- Corrected onsite. Container was marked "Empty" with date it became emptied. <br />45. Emergency phone number for SJC OES is outdated. A emergency phone number sheet posted on <br />a wall near the south wall had an outdated phone number for the SJC OES. The contingency plan must <br />include the following information must be posted by a phone: the name and phone number of the <br />emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; <br />and the phone number of the fire department, and local agencies unless the facility has a direct alarm. <br />Correct the emergency contact phone number for SJC OES. <br />- Corrected onsite. Phone number is outdated. <br />49. Training records incomplete. Two employee handlers for hazardous waste are receiving formal <br />hazardous waste training annually, however the training logs/ records are missing the job title for the <br />employees. Training records shall be maintained on site until closure of the facility and shall include <br />- name & job title of each employee, <br />- written job description for each position (job title) listed, including requisite skill, education, <br />qualifications, and duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />Correct by including training log to include the above information. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTEDDATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION Y T ME A D'S CURRENT HOURLY RATE. <br />EHD Ins ector, <br />Re <br />Title: <br />/I v <br />EHD 23-02-003 ev 08/10/10 CONTINUATION FORM <br />
