In order to protect the ground water <br />supply and your water source from potential degradation, please take the associated corrective action within <br />30(thirty) days as of the date of this letter. <br />❑x Lack of or Inadequate Concrete Platform around Wellhead. <br />(SJC Water Well Standards, Section 4.10. — Surface Seal) <br />Corrective Action: Repair existing or install a new concrete platform around the wellhead. <br />❑ Lack of or Inadequate Sanitary Seal at Wellhead. <br />(SJC Ord. Code, Title 9, Section 9-1115.5. — Well Regulations) <br />Corrective Action: Install a sanitary seal between casing and well cover plate/seal all gaps and <br />openings. <br />❑ Lack of or Inadequate Sampling Tap. <br />(SJC Water well Standards, Section 4.13. — Sampling Tap) <br />Corrective Action: Install a non -threaded sampling tap between the wellhead and the check valve <br />or within three (3) feet of the well head. <br />❑ Lack of or Inadequate Backflow Prevention. <br />(SJC Water Well Standards, Section 4.14. — Backflow Prevention) <br />Corrective Action: Install an approved protective device to prevent backflow and/or back <br />siphonage into a well. <br />❑ Sounding Tube or Air Vent not Sealed. <br />(SJC Water Well Standards, Section 4.13. — Sounding Tube/Air Vent Pipe) <br />Corrective Action: Sounding Tube or Fill Pipe -Install a watertight, threaded cap at the upper end <br />of the pipe. Air Vent -extend pipe 12 inches above with end inverted in a <br />downward direction and covered with 16 -mesh screening. <br />❑ Excessive Vegetation or debris around Wellhead. <br />(SJC Ord.
