SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 600 East Main Street, Stockton, California 95202-3029 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sigov.org/ehd/unitiii.html <br /> CONTINUATION FORM Page: 3 of 4 <br /> OFFICIAL INSPECTION REPORT Date: 04/15/11 <br /> Facility Address: 1538 Nellis St., Ste C, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II, <br /> This is a report of the Routine hazardous waste inspection conducted at the referenced facility by San <br /> Joaquin County Environmental Health Department (EHD). The following are Minor Violations - Notice to <br /> Comply: 7,8,79/83/84. <br /> 7. One 5-gallon container of waste oil outside the shop require hazardous waste determination. <br /> -- Generator shall make hazardous waste determination of the waste and manage accordingly. <br /> * Immediately make a hazardous waste determination of the above wastes and manage each <br /> accordingly, or dispose of the waste as hazardous waste in accordance to California Code of <br /> Regulations (CCR) Title 22. Submit a copy of disposal receipt to EHD by May 16, 2011. <br /> 8. This facility does not have a valid EPA ID number to manage hazardous waste. <br /> -- A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, <br /> hazardous waste without an EPA ID number. <br /> * Immediately obtain an EPA ID number to manage hazardous waste and submit evidence to the EHD <br /> by May 16, 2011. A form has been provided. <br /> 79,83,84. Six Universal Waste fluorescent lamps not in a container in the storage room, and one in the <br /> paint booth outside of the shop in the backyard. <br /> -- Universal Wastes shall be stored in a closed container that prevents breakage and release to the <br /> environment. The container shall be labeled to identify the type of Universal Waste and marked the <br /> initial date of accumulation. Universal Waste shall be managed within one year of storage and retained <br /> on site a copy of the disposal receipt for at least 3 years. Please be aware that Universal Waste includes <br /> but not limited to lamps, alkaline batteries, mercury containing switches, CRT, non-empty aerosol cans, <br /> and electronic devices. <br /> * Immediately store and label the above Universal Waste - lamps in a container that prevents <br /> breakage and release to the environment, and manage and retain disposal records on site as per this <br /> section. A pamphlet for the Small Hazardous Waste Disposal Facility has been supplied to the <br /> generator during this inspection. <br /> NOTES <br /> The referenced facility began business in late December 2010, and has not generated any measurable <br /> hazardous waste in 2010. A copy of Auto Body Best Management Practice factsheet, CUPA class <br /> schedule, blank hazardous waste labels, hazardous waste labeling factsheet, and San Joaquin County <br /> Small Business Hazardous Waste Disposal Program pamphlet, have been provided to generator during <br /> this inspection. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUB CT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD pect HD Title: <br /> "f— 0�� 7 <br /> EHD 23-02-003 R 08/10/10 CONTINUATION FORM <br />
