Both of these documentations must be submitted annually to <br />EHD. You told me I can email you these forms which I have attached. Please read Dollars & Sense: Financial <br />Responsibility Requirements for UST as it will help you on the amount of financial coverage you need at this <br />website http://www.epa.gov/oust/pubs/dol&sens.pdf <br />If you are unable to open these attachments, they can be found at our department website <br />http://www.sjgov.org/ehd/Programs/Unitlll/CFOLetter.pdf (Letter from Chief Financial Officer) and <br />http://www.sjgov.org/ehd/Programs/Unitlll/Financial_Responsibility_Form_l .pdf (Certification of Financial <br />Responsibility). <br />Submit both forms by 03-26-10 along with requested documents, a copy of Return to Compliance Certification (on <br />the backside of the inspection report), and statements how you will correct violations 34, 7, 11, 12, 23c5 that were <br />written on your routine UST inspection report. <br />I have also attached a file on free courses taught at our department. I strongly suggest you and another store <br />employee to attend the course on UST-owner/operator. It is a four hour course which will benefit your facility in <br />complying with underground storage tank regulations.
