Underground Storage Tanks — Tank Pages — Form B — Sign and date the bottom <br />of each form. <br />3. Board of Equalization UST Storage Fee Account Number — Call (916) 322-9669 <br />to obtain a current number for this facility. <br />4. Information and Instructions for Obtaining A Permanent California ID Number: <br />Fill out the application for a California Hazardous Waste Permanent ID Number <br />and fax it to (916) 255-4703. <br />5. Certification of Financial Responsibility/Letter from Chief Financial Officer: Fill <br />out the form provided and make a copy for our records. This document is <br />required to be submitted annually. <br />6. Underground Storage Tank Monitoring and Response Plan: Fill out and submit a <br />copy. Retain onsite for future reference by employees. <br />7. Hazardous Materials Management Plan: Contact the Office of Emergency <br />Services, Mr. Dennis Fields, at 468-3969 for information on updating this plan. <br />8. Report #5021: Fill out new information and sign and date. Bring with all other <br />forms to our accounting office to pay for transfer fees. <br />9. Also enclosed is information on the "Designated UST Operator".
