Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Complete and submit a copy of the financial responsibility to the EHD within 30 days of receiving this report. 105. UST facility and tank forms on file with the EHD are not current. Any change of information on these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the facility and tank forms to the EHD. Complete a separate tank form for each tank. 106 & 202. Current approved copies of the monitoring and response plans were not found on site. The monitoring and response plans must be current and approved by the EHD. Complete and submit a copy of the monitoring and response plans for approval by the EHD within 30 days of receiving this report. An approved copy will be returned for your records. 108. Secondary containment testing was last performed on 1/23/09 and was due by 1/2312. Testing has not been completed and is 5 months past due. Secondary containment testing is required once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD. 109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing were last performed on 2/25/11 and was due by 2/25/12. Testing has not been completed and is 4 months past due. These tests are required once every 12 months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly rate. 113. Facility lack designated operator. By January1, 2005, owners of UST systems shall submit a signed statement to the local agency indicating that the owner understands and is in compliance with the applicable UST requirements, and identifying the designated UST operator for each facility owned. Submit notification to the EHD identifying all the designated operators for this facility immediately. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
