PLAN REVIEW AND OPERATING PERMIT FEES <br /> 1. New facility plan review fee of $1,216.00 (8 hour minimum per facility). Also due at <br /> time of plan submittal: $641.00 facility fee, $152.00 per tank fee plus $20.00 state <br /> surcharge fee per tank. Facilities not already on inventory in a CUPA program have <br /> an additional $49.00 per site state surcharge. <br /> 2. New installation of an additional tank at an existing facility is $1,216.00 plan review <br /> fee(8 hour minimum per facility) plus$152.00 per new tank, and $20.00 per new tank <br /> state surcharge fee. <br /> 3. It should be noted that a $1,216.00 plan review check fee required for each submittal <br /> allows for up to eight(8) hours of staff review and/or inspection time. Additional time <br /> spent on plan review, meetings, inspections, phone or office consultations will be <br /> billed to the applicant at a rate of$152.00 per hour. <br /> 4. The maximum review time for UST installation plans is 40 working days. A preliminary <br /> review will be conducted by the EHD within 20 working days of submittal date to <br /> identify any gross plan deficiencies. <br /> 5. If gross deficiencies are identified, the plans will be returned with a checklist <br /> identifying plan deficiencies. <br /> 6. To expedite the review of installation plans, time and a half per hour overtime fee may <br /> be submitted to have review completed within 10 working days of submittal date if staff <br /> is available to perform the review. <br />
