This work was completed on June 29, 2015. The <br /> permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. Since this sensor was tested during the most recent monitoring system certification which <br /> occurred on 6-08-2016, no further testing is required at this time. <br /> 4. Qualified service technicians must possess or work under the direct and personal supervision of <br /> an individual physically present at the work site who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the Contractors <br /> State License Board (may be issued to the installation company), or a Tank Tester's license <br /> issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the International Code <br /> Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> tested, repaired, or serviced. <br /> 5. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, and <br /> b. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> installed, and <br /> c. A valid, current UST Installation/Retrofitting certificate issued by the International Code <br /> Council (ICC). <br /> 6. The tank owner or operator shall log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/and ensure all UST information is current and accurate. <br />
