The following is an itemized list of underground storage tank violations that have not <br />been addressed for FAST N ESY as of September 01, 2015. <br />Open violations from June 11, 2015 inspection <br />Violation #123 - Permit to repair UST system not obtained. <br />One of the turbine relays was replaced on 2-12-2015 and a permit was not obtained from the EHD (per Donlee Puml <br />work order, no indication as to which product relay was replaced was provided). Emergency repairs relating to UST <br />system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor <br />pursuant to the EHUS policy and a permit obtained within one business day from the EHD. All other repairs must <br />have a permit submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the <br />EHD for the replaced component. A "Failure to Obtain a Permit" penalty applies. <br />Page 1 of 1 <br />
